About the Role

An exciting opportunity to join a great team at a Foundation NHS Trust in the North East. 

As the Trust expands key services and frailty pathways to benefit patients, they are developing and enhancing the service across sub-specialties, including; Acute Frailty (SDEC), Acute Frailty Units, Frailty Specialist Units, Movement Disorder Services, Ortho-Geri's, Surgical Liaison, Hospital at Home and Outpatient activity.

Main duties of the job

The basic whole-time contract is for 10 programmed activities; 8.5 DCC (7 clinical sessions with 1.5 PAs admin time and for educational supervision) + additional 1.5 SPA. Consideration for part time working could be accommodated.

There will be a discussion with candidates regarding the final job plan/oncall arrangements, prior to the interview and appointment to ensure the job plan reflects both the individuals and organisation goals. On calls can be solely in geriatric medicine or potentially GIM depending on preference.

Additional programmed PAs may be available for additional activities and specialist interests if desired. Additional SPA time may be available depending on individual and service requirements.

Requirements include:
- GMC registration with a full licence to practice
- MRCP 
- Good clinical skills including the management of acutely unwell medical patients and up to date knowledge in geriatric medicine.

Job Summary We are seeking a dynamic and forward-thinking Consultant Physician in Geriatric Medicine to join a multi-disciplinary team. This position is open to both newly trained and experienced consultants who are passionate about developing and delivering innovative care models for frail elderly patients. The successful candidate will work closely with the established team to enhance the acute frailty pathway and provide high-quality care within a 32-bed elderly care ward. Additionally, you will contribute to rapid access frailty clinics, delivering Comprehensive Geriatric Assessments (CGA) and collaborating with community services to reduce unplanned admissions. This role includes participation in the out-of-hours and on-call rota (1:12). Main Duties and Responsibilities As part of a collaborative team, you will: Lead and contribute to the development of acute frailty pathways and associated services. Deliver care for patients on the Care of the Elderly ward, ensuring adherence to high clinical standards. Provide early access CGA in rapid access and ‘hot’ clinics to support primary care and reduce emergency admissions. Offer advice and guidance to community GPs. Participate in the education and supervision of Junior Doctors, Medical Students, and multidisciplinary team members. Support service efficiency through administrative tasks, business planning, and participation in the booked appointment system.
