Job Summary
We are seeking a dynamic and forward-thinking Consultant Physician in Geriatric Medicine to join a multi-disciplinary team. This position is open to both newly trained and experienced consultants who are passionate about developing and delivering innovative care models for frail elderly patients.
The successful candidate will work closely with the established team to enhance the acute frailty pathway and provide high-quality care within a 32-bed elderly care ward. Additionally, you will contribute to rapid access frailty clinics, delivering Comprehensive Geriatric Assessments (CGA) and collaborating with community services to reduce unplanned admissions.
This role includes participation in the out-of-hours and on-call rota (1:12).
Main Duties and Responsibilities
As part of a collaborative team, you will:
Lead and contribute to the development of acute frailty pathways and associated services.
Deliver care for patients on the Care of the Elderly ward, ensuring adherence to high clinical standards.
Provide early access CGA in rapid access and ‘hot’ clinics to support primary care and reduce emergency admissions.
Offer advice and guidance to community GPs.
Participate in the education and supervision of Junior Doctors, Medical Students, and multidisciplinary team members.
Engage in clinical meetings, research projects, and postgraduate activities within the Trust.
Support service efficiency through administrative tasks, business planning, and participation in the booked appointment system.
Collaborate with other specialist teams to ensure integrated care delivery.
Participate in clinical governance, audits, and trials to drive service improvement.
Complete mandatory training and professional development, including annual appraisals and job plan reviews.

Requirements include
MRCP or equivalent qualification.
GMC registration with a full licence to practice
Demonstrable experience and competence in Geriatrics and GIM.
